Welcome to Elant at Fishkill.

Elant at Fishkill, a 160-bed nursing home and rehabilitation center, has been part of the community for more than 32 years, and offers many unique and award-winning programs including a specialized dementia care unit, a comprehensive rehabilitation clinic for short-term, long-term, outpatient and home care rehabilitation, an outstanding therapeutic activity program and volunteer program.

Elant at Fishkill specializes in compassionate care and expert rehabilitation. We try to make a stay in our facility as pleasant and productive as possible, offering intensive rehabilitation therapy. Our experienced clinicians and therapists can help residents transition quickly from hospital to home through occupational, physical and speech therapies. We have a rehabilitative program that focuses on helping people recover their abilities to the fullest extent possible by using a variety of techniques and a completely individualized plan of care. If a resident is planning to return home, the rehabilitation team will perform a home evaluation to assess safety needs and make suggestions for necessary equipment and continued care through therapy at home with our long-term home health care services.

Elant at Fishkill is known for its broad spectrum of stimulating creative and mind sharpening activities designed to continue previous life experiences and provide new opportunities for residents. Regularly scheduled activities include ceramics and craftwork, fitness and cooking groups, gardening, concerts, socials, music, massage and pet therapies, visitation, ethnic programs, social clubs, discussion groups and frequent outings to local restaurants, stores, theaters, and local attractions.

Elant at Fishkill woman smilingFor more than 20 years, Elant at Fishkill has provided dementia care for our residents with Alzheimer’s Disease and related illnesses. The M.I.N.D. Unit (Maintaining Independence in Neurocognitive Disorders) is secure and provides a safe environment for those with cognitive and memory impairment. Specially trained staff offer compassionate care and supervision, and assist in dealing with memory loss, orientation problems and impaired social and language skills. Residents enjoy self esteem enhancing activities such as reminiscing groups, dancersize sessions, musical programs, pet visits, cooking, word games, community outings and intergenerational activities.

Founded in 1989, Fishkill Health Center’s Foster Family Care Program matches elderly or disabled adult clients with families who share their homes and provide personal care for the client in the stimulating and nourishing environment of family life.

The program is ideal for elderly or disabled adults who do not need total care, but do require 24-hour supervision and cannot get that care at home. For qualified clients, the FHC Foster Family Care Program is an alternative to residential or nursing home care.

Foster Families and their homes are carefully screened for suitability and family members are thoroughly trained in the needs of clients before being accepted into the program. Compatibility between the client and Foster Family is paramount in making the match.

Caregivers receive payment for providing room and board and personal care services, and enjoy the many personal satisfactions of living along-side an older, uniquely experienced person. FHC’s Foster Family Care Program affords both clients and caregiver families the opportunity for fuller, richer lives.

Elant at Fishkill handsWho Are Foster Family Care Clients?
  • Foster Care candidates must be eligible for residential care and not have access to a suitable home in the community.
  • Candidates must be either 65 years old or older, or must be physically disabled adults 18 years old or older.
  • Candidates must have some medical needs and a medical diagnosis.
  • Candidates require 24-hour supervision but do not need total care.
  • Referrals of candidates are usually made of immediate family members, hospitals, adult homes or the Department of Social Services, OMR and Mental Health Agencies.
Who are the Foster Family Caregivers?
  • Foster Families may be couples with or without children, single parents or adults living alone or with others.
  • Caregivers are professional nurses who are currently inactive, qualified personal care workers, aides or home health aides or persons who are willing and able to be trained as qualified personal care workers.
  • In the application process, prospective Foster Families are carefully screened, including references, and a home visit is conducted. Details such as bathroom accommodations, wheelchair accessibility, closet space, smoke detectors and other safety features in the house are checked by Fishkill Health Center staff. The family's home is also inspected and certified by the Department of Social Services.
What is required of Foster Families?
  • Foster Families must live in a house or apartment with safe accommodations for one, two or three clients.
  • Foster Families must provide 24-hour supervision for the client. Providers must also have a qualified back-up person to stand in when necessary.
  • The Foster Family must have another source of income to sustain the household and the primary caregiver must not have a job outside the home.
  • The caregiver must have a yearly physical.

Elant at Fishkill groupWhat do Foster Family Care clients receive?
  • Clients enjoy a home with a caring family with room, board and personal care from a Foster Family caregiver.
  • Clients receive scheduled visits by a registered nurse for services and care management in accordance with Physician's Orders. Comprehensive Long Term Home Health Care services are available if needed.
  • Clients may attend peer-centered group up to five times a week if eligible.
  • Clients receive a monthly personal allowance through the Social Security Administration when eligible.
  • Assistance with family/ client relations is available to clients at all times.
What do foster families receive?
  • Foster Family caregivers get a monthly payment for both room and board and providing personal care services for each client.
  • Foster Family caregivers are trained/certified as Personal Care Workers. They receive continued inservice education yearly. Training includes fire/ safety, infection control, first aid, patients' rights and communication skills.
  • Foster Families are eligible for up to 2 weeks a year respite from the foster care responsibilities.
Elant at FishkillLong Term Home Health Care Programs (LTHHCP)

One of only two such programs in the county, LTHHCP provides comprehensive care in the comfort and privacy of the client's own home. The program is open to anyone in Dutchess County eligible for care in a residential care facility or nursing home.

Admission is based on the results of a detailed assessment, after which a comprehensive care plan is developed by the primary nurse who coordinates the various services. The program is long term in that it is designed to help clients stay in their homes for as long as possible.

LTHHCP is covered by Medicaid, Medicare or private pay provided that the amount of service needed is within 75 percent of what it would cost to be in the local nursing home.

Short term skilled care for nursing, physical, occupational and speech therapies can be offered in the comfort of your own home. We continue your treatment plan post-discharge from a hospital. Our professional staff will assist you in your rehabilitation and recuperation in your home setting. We also evaluate your home and make recommendations for any environmental adaptations.

HIV Home Care Program

A unique program in Dutchess County specifically for adults who are diagnosed with HIV-related illnesses. Like LTHHCP, the HIV Home Care Program provides comprehensive care in the client's home.

But in Dutchess County, home care can also be where the help is for the elderly or disabled adult in need of daily health care. Elant at Fishkill offers three excellent programs that help individuals remain as independent as possible in the home.

An alternative to residential nursing home care, Elant at Fishkill's programs foster a sense of freedom, individuality and personal dignity. They differ from other programs in that they are uniquely comprehensive and individualized. Each client's care plan is created just for them. The plan considers the medical, social, psychological and environmental aspects of his or her situation. To maximize the care, family and support systems play an important role in designing the plan. This "creative care management" is the primary focus of each Elant at Fishkill program.

Our award winning volunteer program is an example of how our residents maintain their connections to the community.  Local musicians entertain regularly while churches, spiritual groups and school visits provide intergenerational community connections.
