Elant Choice - Managed Long - Term Care Plan
Elant Choice is an innovative, community- based managed long-term care plan that enables residents of Orange County, aged 18 or older, to receive care while remaining at home, in their community. Elant Choice enrollees have access to an affordable and coordinated system of care that provides much needed health care and social services in the most appropriate setting.
Elant at Home - Companion CareElant at Home provides companion and support services for people who require non medical assistance in their home by providing dependable and affordable services including companion services, light and heavy housekeeping, shopping/errands, household financial assistance and personal care.
Licensed Home Health CareElant's Licensed Home Health Services are designed to meet health, social, emotional and environmental needs while enhancing the quality of life within the limitations of acute and chronic illnesses. This is done through compassionate, dependable and dignified care within the comfort of one's own home and familiar surroundings.
- Skilled nurses who provide specialized treatments and procedures, assess vital signs, monitor and report symptoms and instruct the patient and family regarding care at home.
- Home Health Aides or Personal Care Aides to assist with personal care needs.
- Physical Therapists who provide exercise programs, equipment and prosthetic device training.
- Speech Therapy
- Occupational Therapy
- Comprehensive Case Management
The Long Term Health Care Programs provide comprehensive care in the comfort and privacy of the client's own home. The program is open to anyone in Dutchess and Orange Counties eligible for care in a residential care facility or nursing home. The Long Term Home Health Care Program is covered by Medicaid, Medicare or private pay provided that the amount of service needed is within 75% of what it would cost to be in a nursing home.
